Case Manager - care planning (remote)

Westbrook Glen Health runs clinics and patient services around El Paso, and we need a Case Manager to lead our care planning. You would join a small organisation of about 181 people, working mostly from home. What you will do • Help with applications and appointments for insurance authorisations. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in El Paso. • Applications are reviewed on a rolling basis.
