Case Manager - care planning (remote)

We are growing our team at Northwind Cross Health Group and adding a Case Manager whose main responsibility will be care planning. Clinicians and administrators work side by side. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for electronic records. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Remote-friendly, with quarterly visits in Atlanta. • Applications are reviewed on a rolling basis.
