Case Manager - care planning (remote)

Fernwood Cobalt Community Partners is looking for a Case Manager. The job centres on care planning for benefits enrolment, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for benefits enrolment. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Part-time, about 24 hours a week, afternoons. • Fully remote, with a monthly team day in Fresno. • We reply to every applicant.
