Case Manager - care planning (remote)

We are growing our team at Northwind Nest Community Partners and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for benefits enrolment. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Contract for three months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in New Orleans. • Applications are reviewed on a rolling basis.
