Case Manager - care planning (remote)

Silverline Pike Learning Group is an education provider in Rochester serving students and families. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for attendance records. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with attendance records. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Rochester. • Applications are reviewed on a rolling basis.
