Case Manager - care planning (remote)

Amberfield Junction Community Partners is a nonprofit in Colorado Springs supporting families with food, housing and job help. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 774 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for volunteer shifts. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Contract for four months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Colorado Springs. • We will tell you what to expect at each step of the hiring process.
