Case Manager - care planning (remote)

Foxglove Corner Early Learning runs childcare centres in Providence caring for infants, toddlers and preschoolers. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for meal programmes. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with meal programmes. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with clear deliverables from the first week. • Fully remote, with a monthly team day in Providence. • Applications are reviewed on a rolling basis.
