Case Manager - care planning (remote)

Summit Analytics provides shared administrative services to local governments around Jacksonville, and we need a Case Manager to lead our care planning. We follow procedures carefully and explain them clearly. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for budget submissions. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with budget submissions. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Remote-friendly, with a monthly team day in Jacksonville. • We reply to every applicant.
