Case Manager - care planning (remote)

Bluepeak Keystone Civic Services is looking for a Case Manager. The job centres on care planning for public records, and it would suit someone who likes steady, practical work. We follow procedures carefully and explain them clearly. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for public records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with public records. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with quarterly visits in Cincinnati. • We reply to every applicant.
