Case Manager - care planning (remote)

Harborlight Stone Care Services Group supports families and individuals in El Paso with personal care and coordination, and we need a Case Manager to lead our care planning. You would join a small organisation of about 144 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client records. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • Fully remote, with an occasional day on site in El Paso. • We reply to every applicant.
