Case Manager - care planning (remote)

Summit Group manages apartment communities around Minneapolis, and we need a Case Manager to lead our care planning. Residents are neighbours, not numbers. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease renewals. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with lease renewals. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Minneapolis. • We reply to every applicant.
