Case Manager - care planning (remote)

Silverline Fox Early Learning Partners is looking for a Case Manager. The job centres on care planning for classroom supplies, and it would suit someone who likes steady, practical work. You would join a large organisation of about 736 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for classroom supplies. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with classroom supplies. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with an occasional day on site in Jacksonville. • We reply to every applicant.
