Case Manager - care planning (remote)

Foxglove Junction Community Partners is looking for a Case Manager. The job centres on care planning for volunteer shifts, and it would suit someone who likes steady, practical work. You would join a large organisation of about 768 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for volunteer shifts. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Contract for six months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Detroit. • Applications are reviewed on a rolling basis.
