Case Manager - care planning (remote)

Rivermark Harbor Care Services is looking for a Case Manager. The job centres on care planning for visit scheduling, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for visit scheduling. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Remote-friendly, with quarterly visits in Providence. • We will tell you what to expect at each step of the hiring process.
