Case Manager - care planning (remote)

Foxglove Junction Community Partners is a nonprofit in Detroit supporting families with food, housing and job help. We are hiring a Case Manager to take on care planning. Mission matters here, and so does looking after staff. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for grant reporting. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Fully remote, with an occasional day on site in Detroit. • We will tell you what to expect at each step of the hiring process.
