Case Manager - care planning (remote)

We are growing our team at Bright Haven Learning Group and adding a Case Manager whose main responsibility will be care planning. Families are partners. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for student accounts. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with student accounts. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • Remote-friendly, with quarterly visits in Pittsburgh. • We reply to every applicant.
