Case Manager - care planning (remote)

Stonegate Wilder Learning Group is looking for a Case Manager. The job centres on care planning for family communication, and it would suit someone who likes steady, practical work. You would join a small organisation of about 139 people, working mostly from home. What you will do • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication.
