Case Manager - care planning (remote)

Summit Rook Housing manages affordable housing communities across New Orleans. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease compliance. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • Remote-friendly, with an occasional day on site in New Orleans. • We will tell you what to expect at each step of the hiring process.
