Case Manager - care planning (remote)

Lumen Birch Care Services Partners is a social-care provider serving older adults and people with disabilities around Charleston. We are hiring a Case Manager to take on care planning. The work is personal and so is the culture. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for client records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Fully remote, with quarterly visits in Charleston. • We reply to every applicant.
