Case Manager - care planning (remote)

Highbridge Haven Learning Group is looking for a Case Manager. The job centres on care planning for student accounts, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for student accounts. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with student accounts. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with a monthly team day in Dallas. • We will tell you what to expect at each step of the hiring process.
