Case Manager - care planning (remote)

Cedar North Learning Group operates schools and learning programmes around Houston. We are hiring a Case Manager to take on care planning. Families are partners. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for learning materials. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Remote-friendly, with an occasional day on site in Houston. • We reply to every applicant.
