Case Manager - care planning (remote)

Redstone Drift Health is looking for a Case Manager. The job centres on care planning for electronic records, and it would suit someone who likes steady, practical work. We are busy and kind, and we look after each other. What you will do • Help with applications and appointments for electronic records. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Part-time, about 28 hours a week, with flexible days. • Fully remote, with an occasional day on site in Tulsa. • We reply to every applicant.
