Case Manager - care planning (remote)

Cedar Vista Learning Group runs classrooms and tutoring across the Portland area. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 36 people, working mostly from home. What you will do • Connect clients with partner agencies. • Help with applications and appointments for attendance records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with attendance records. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with a monthly team day in Portland. • We will tell you what to expect at each step of the hiring process.
