Case Manager - care planning (remote)

Fernwood Meadow Housing manages affordable housing communities across Tulsa. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for rent assistance. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with rent assistance. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in Tulsa. • We will tell you what to expect at each step of the hiring process.
