Case Manager - care planning (remote)

Highbridge Health is looking for a Case Manager. The job centres on care planning for resident events, and it would suit someone who likes steady, practical work. You would join a small organisation of about 39 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for resident events. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with resident events. • Bilingual English and Spanish. Working arrangement • Contract for three months, with clear deliverables from the first week. • Remote-friendly, with quarterly visits in Tampa. • We reply to every applicant.
