Case Manager - care planning (remote)

Silverline Sparrow Care Services is looking for a Case Manager. The job centres on care planning for licensing, and it would suit someone who likes steady, practical work. You would join a small organisation of about 30 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for licensing. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with an occasional day on site in Savannah. • Applications are reviewed on a rolling basis.
