Case Manager - care planning (remote)

We are growing our team at Rivermark Quarry Health and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of about 371 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for referrals. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • Fully remote, with a monthly team day in Pittsburgh. • Applications are reviewed on a rolling basis.
