Case Manager - care planning (remote)

Bright Linden Health Group provides primary and specialty care in Buffalo. We are hiring a Case Manager to take on care planning. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with quarterly visits in Buffalo. • We will tell you what to expect at each step of the hiring process.
