Case Manager - care planning (remote)

Harborlight Hollow Care Services Group is a social-care provider serving older adults and people with disabilities around Atlanta. We are hiring a Case Manager to take on care planning. The work is personal and so is the culture. What you will do • Help with applications and appointments for licensing. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Contract for four months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Atlanta. • We will tell you what to expect at each step of the hiring process.
