Case Manager - care planning (remote)

Bright Fern Care Services is looking for a Case Manager. The job centres on care planning for licensing, and it would suit someone who likes steady, practical work. We train and promote from within. What you will do • Help with applications and appointments for licensing. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Denver. • We reply to every applicant.
