Case Manager - care planning (remote)

We are growing our team at Foxglove Junction Community Partners Group and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 21 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for food distribution. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Charleston. • We reply to every applicant.
