Case Manager - care planning (remote)

Highbridge Nest Learning Group runs classrooms and tutoring across the Colorado Springs area. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for attendance records. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with attendance records. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • Fully remote, with a monthly team day in Colorado Springs. • Applications are reviewed on a rolling basis.
