Case Manager - care planning (remote)

Highbridge Stone Care Services supports families and individuals in Buffalo with personal care and coordination. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 85 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for caregiver training. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, with flexible days. • Remote-friendly, with a monthly team day in Buffalo. • We will tell you what to expect at each step of the hiring process.
