Case Manager - care planning (remote)

We are growing our team at Highbridge Sable Housing and adding a Case Manager whose main responsibility will be care planning. Collaboration with local agencies is constant. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for lease compliance. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with lease compliance. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Fully remote, with an occasional day on site in Portland. • We reply to every applicant.
