Case Manager - care planning (remote)

Northwind Bridge Care Services is looking for a Case Manager. The job centres on care planning for visit scheduling, and it would suit someone who likes steady, practical work. Teams support each other, especially on hard days. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for visit scheduling. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling.
