Case Manager - care planning (remote)

Westbrook Junction Housing Group is looking for a Case Manager. The job centres on care planning for waiting lists, and it would suit someone who likes steady, practical work. Collaboration with local agencies is constant. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for waiting lists. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with waiting lists. • Bilingual English and Spanish.
