Case Manager - care planning (remote)

We are growing our team at Fernwood Ridge Learning Group and adding a Case Manager whose main responsibility will be care planning. We follow the school calendar and we like people who like learning. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for student accounts. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with student accounts. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with quarterly visits in Boston. • We reply to every applicant.
