Case Manager - care planning (remote)

Highbridge Meridian Health Group is looking for a Case Manager. The job centres on care planning for clinic supplies, and it would suit someone who likes steady, practical work. Clinicians and administrators work side by side. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Remote-friendly, with quarterly visits in Sacramento. • Applications are reviewed on a rolling basis.
