Case Manager - care planning (remote)

We are growing our team at Harborlight Orchard Care Services and adding a Case Manager whose main responsibility will be care planning. Teams support each other, especially on hard days. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for licensing. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with an occasional day on site in Boston. • We will tell you what to expect at each step of the hiring process.
