Case Manager - care planning (remote)

Summit Property Management manages apartment communities around Columbus, and we need a Case Manager to lead our care planning. Residents are neighbours, not numbers. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for resident events. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with resident events. Working arrangement • Part-time, about 24 hours a week, with flexible days. • Fully remote, with quarterly visits in Columbus. • Applications are reviewed on a rolling basis.
