Case Manager - care planning (remote)

Lumen Fox Early Learning is looking for a Case Manager. The job centres on care planning for classroom supplies, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for classroom supplies. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with classroom supplies. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Remote-friendly, with quarterly visits in Atlanta. • Applications are reviewed on a rolling basis.
