Case Manager - care planning (remote)

Stonegate Iron Learning Group is looking for a Case Manager. The job centres on care planning for learning materials, and it would suit someone who likes steady, practical work. You would join a small organisation of about 189 people, working mostly from home. What you will do • Help with applications and appointments for learning materials. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • Fully remote, with a monthly team day in Buffalo. • We will tell you what to expect at each step of the hiring process.
