Case Manager - care planning (remote)

Silverline Thistle Property Management is looking for a Case Manager. The job centres on care planning for resident events, and it would suit someone who likes steady, practical work. We are flexible about schedules. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for resident events. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with resident events. Working arrangement • Contract for four months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Albuquerque. • We will tell you what to expect at each step of the hiring process.
