Case Manager - care planning (remote)

Bluepeak Corner Learning Group operates schools and learning programmes around Chicago, and we need a Case Manager to lead our care planning. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for learning materials. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with learning materials. Working arrangement • Contract for six months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Chicago. • Applications are reviewed on a rolling basis.
