Case Manager - care planning (remote)

We are growing our team at Stonegate Pike Learning Group and adding a Case Manager whose main responsibility will be care planning. We follow the school calendar and we like people who like learning. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for school calendars. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with an occasional day on site in Columbus. • We will tell you what to expect at each step of the hiring process.
