Case Manager - care planning (remote)

Ironwood Ivory Health provides primary and specialty care in Wichita, and we need a Case Manager to lead our care planning. Continuing education is supported. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for clinic supplies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Fully remote, with a monthly team day in Wichita. • Applications are reviewed on a rolling basis.
