Case Manager - care planning (remote)

We are growing our team at Westbrook Keystone Civic Services and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of about 759 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for constituent requests. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with constituent requests. Working arrangement • Part-time, about 20 hours a week, with flexible days. • Remote-friendly, with quarterly visits in Cincinnati. • We will tell you what to expect at each step of the hiring process.
