Case Manager - care planning (remote)

We are growing our team at Greenridge Willow Early Learning and adding a Case Manager whose main responsibility will be care planning. Low staff turnover is a goal we take seriously. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for tuition and subsidies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with tuition and subsidies. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Fully remote, with quarterly visits in Raleigh. • We will tell you what to expect at each step of the hiring process.
