Case Manager - care planning (remote)

Bright Ashton Civic Services is looking for a Case Manager. The job centres on care planning for constituent requests, and it would suit someone who likes steady, practical work. Public service means patience, paperwork and pride in the outcome. What you will do • Help with applications and appointments for constituent requests. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with constituent requests. • Bilingual English and Spanish.
