Case Manager - care planning (remote)

Westbrook Bright Community Partners is a Raleigh charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on care planning. We are small enough to know everyone's name. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • Fully remote, with a monthly team day in Raleigh. • Applications are reviewed on a rolling basis.
